ASPECTS THAT AN EMERGENCY PLAN MUST CONTEMPLATE Every Plan must include: the corporate name, the type of establishment, name and qualifications of the Technical Manager and the Permit Holder, location and exact address of the establishment, telephone numbers, fax, postal box, email, work hours (administrative hours, shift hours), number of workers, indicating quantity by sex and shift, and finally, the letters signed by the professional in charge of preparing said document, that of the Technical Manager, and of the Permit Holder will be attached, indicating the responsibility both in the suitability of what is expressed in said documents and their commitment to implementation and compliance.
I. STAGE
1. ANALYSIS OF THE LEVEL OF THREATS AND VULNERABILITY It is based on observation, recognition, and study of the risk conditions that the workplace presents, both internally and externally, to determine the potential dangers and those situations that could be considered special during an emergency; whether due to their consequences or damages they may cause to people, facilities, and systems. This stage allows stakeholders to have the necessary material to propose corrective measures to face such risks. (Include a section on the historical background of past events and their impact on the property and incorporate the primary aspect of preserving life above all activities being carried out).
1. 1. EXTERNAL LEVEL It consists of determining the type of threats that exist in the area where the establishment is located, whether: landslides, seismic faults, proximity to rivers or other water sources, adjacent industrial zones, high-tension lines, and hazardous material storage; everything that may affect the normal functioning of the workplace or endanger the lives of its occupants is considered. (The use of access routes in emergency cases must be especially considered).
In this analysis, the location of emergency response or attention bodies such as the Fire Department (Bomberos), Red Cross (Cruz Roja), INS, and others that could be used during an event must be considered to determine the times it would take them to arrive.
In addition to the procedures for information, communication, and prevention of emergencies to neighbors, with respect to residences, establishments (industries, businesses, services, and others).
I. 2. INTERNAL LEVEL It consists of a general assessment of the workplace where the following is contemplated:
1.2.1 STRUCTURAL CONDITIONS OF THE BUILDING Maintenance status of the structure Size of the building and number of floors Current state of the same (age or years since construction) Characteristics of the materials and techniques used in the construction.
NON-STRUCTURAL CONDITIONS OF THE BUILDING 1.2.2.1 Identification of the construction elements that could be affected during an event, such as curtains, coatings, ceilings, glass, and lamps.
1.2.2.2 Maintenance status of the electrical installations, aqueducts, gas, pipes, tanks, boilers, ovens.
1.2.2.3 Analysis of the space in rooms and work areas: Analyzing location and status of the equipment, as well as an analysis of the location of staff, users, patients, and permanent or temporary companions, if there are space problems or overcrowding, obstructed hallways, as well as determining obstacles, equipment, or machinery that interferes with staff, users, patients, and companions that could constitute a risk condition at the time an event occurs.
1.2.2.4. Assess the location, condition, and capacity of doors, stairs, hallways, and regular and emergency exits, ensuring they are kept unobstructed, especially exits and evacuation routes.
1.2.2.5 It should also contemplate the most dangerous areas of the facility, as well as identify those that present greater safety and can be used as temporary or refuge zones.
1.2.2.6 Conditions of hazardous material storage, characteristics of the stored products, controls and equipment used to attend to emergencies.
1.2.2.7. Analysis of the conditions (quantity, state, type or class, location, and accessibility) of fire protection equipment (extinguishers, hoses, pumps, hydrants, water sources, rescue equipment, first aid, alarm, emergency lights, emergency exits and stairs, safety zones). Indicate the fire prevention measures implemented by the establishment or determine those to be implemented, the brigades (number and characteristics thereof), and any other support material.
II STAGE 2. ADMINISTRATIVE ORGANIZATION OF THE EMERGENCY This consists of the implementation of the organization, components, and functions of the Emergency Committee, which is the structure responsible for coordinating the execution of activities carried out before, during, and after the emergency or disaster.
2.1 Indicate how the Emergency Committee is composed.
2.2 Indicate the name of the Coordinator, of the persons in charge or responsible for the brigades, and any other person that said committee deems necessary; it must have the approval and support of the management and or administration of the establishment, with its functions and responsibilities duly detailed for each of the stages of an emergency.
2.3 Attach the Rapid Response Plans. In these documents, the procedure that will be implemented by the groups or brigades will be indicated, considering the constitution of an Emergency Command, the Fire Prevention and Fighting Brigade, First Aid, Security, Evacuation and Rescue, and Evaluation and Rehabilitation Brigade respectively, and they will contemplate the three stages already mentioned.
2.4 Indicate the Training of the Committee and the respective Emergency Brigades with their respective work schedules.
III.- STAGE 3. EVACUATION AND RESCUE PLAN This evacuation plan must contemplate or consider the information included in Stage I of this guide, considering the number of people who work or remain inside the health establishment (patients, companions, or users). It should be noted that the plan developed below is the procedure that will be implemented within the health establishment; for a better guide, the following is summarized:
Location and signage of safety zones, indicating types of signs, exit routes or access routes to the safety zone.
It must include alternative safety zones, as well as internal or external zones, taking into account the type of event that occurs.
Capacity of the safety zone (taking into consideration the width and position (standing or seated) of the people who will remain within it).
Indicate the exit routes (Indicating the hallways and doors through which people will ascend to the safety zone, quickly and safely, indicating the distribution of personnel who will circulate through them). Identify distances to be traveled and the conditions in which these routes will be kept or how they would be at the time of an event. The movement or flow of people must be indicated by means of arrows, determining their route from their own workplace or the site they occupy until reaching the safety zone. Indicate the prohibition of keeping exits and evacuation routes obstructed.
Signage of dangerous areas such as boiler rooms, general and supply warehouses, fuel deposits, machine rooms, etc., as established by legislation in this field.
Reaction to the alert signal: they must indicate the type of signal that will be used to activate the evacuation plan, at what moment, and how it will function internally within the establishment.
Type of alarm that the establishment will use to make the alert known at the community level or area neighboring the establishment (coordination with the Local Emergency Committee or with the National Emergency Commission (Comisión Nacional de Emergencia) is recommended).
Indicate the type of actions that the establishment will carry out to provide preventive information directed at people who are located inside the building temporarily or occasionally.
Indicate the total Evacuation time for the building, and organization of the respective exits from the different offices or workstations (in those cases where drills have already been carried out).
Identify the radius of action of protection that will be implemented during the response to an emergency, considering both daytime and nighttime hours.
Include the energy backup resources that the building has, such as water, electricity, and fuel reserves, radio communication.
DESIGN OF A SKETCH: The establishment must have the design of a sketch or plan of the establishment in which the previous information is graphically represented so that said diagram is displayed within the health establishment in different sectors, and so that the workers and people who frequent it become familiar with it and so that it can be used at the same time by the Fire Department (Cuerpo de Bomberos), Red Cross (Cruz Roja), and any other entity providing help during the emergency.
If possible, the external location of the health establishment with respect to its communal location will be included in this sketch, considering the location, distances, and travel times of sources of help (Red Cross (Cruz Roja), Fire Department (Bomberos), INS, Emergency Commission, location of water sources, hydrants, among others) indicating the radius of action that will be implemented, as well as the telephone numbers to be used during the event.
IV.- STAGE 4. EVALUATION OF THE PLAN Description of the types of evaluation that will be used to determine the functionality, scope, and limitations of the emergency plan, as well as to determine the corrections required.
Description of the actions that will be carried out for the rehabilitation and reconstruction of the services.
Work schedule (where the dates of conditioning and repairs to be carried out are noted; in order to minimize the vulnerability of the facilities, acquisition, and installation of equipment or systems necessary to face an emergency situation and any other action or activity that originates from the evaluation carried out.

SOP Renewal: applies to all establishments, companies, or businesses that have been granted an SOP and are within one month of its expiration. | | --- | --- | | Box No. 2 Risk group: | Mark with an "x" the box for the risk group to which the activity, for which you are requesting the SOP, belongs. If you carry out other accessory activity(ies) (an activity carried out in an establishment as a complement to its main activity, dependent on the latter and belonging to the same owner or legal representative), they must be indicated. | | Box No. 5 Trade name of the establishment, company, or business requesting the SOP: | Clearly write the trade name of the establishment, company, or business for which you are requesting the SOP. | | Boxes No. 6, 7, 8: | Write down, according to the existing territorial division, the number assigned to the province, canton, and district where the establishment, company, or business is located. | | Box No. 9 Exact address of the establishment (other specific directions): | Clearly write the address of the premises, indicating streets, avenues, name of the neighborhood, and any other detail that allows the establishment, company, or business to be correctly located. | | Boxes No. 10, 11, 12, 13: | Clearly write the telephone numbers, post office box, and email addresses, in order to maintain a timely communication channel with the company when circumstances warrant. | | Box No. 14 Name of the individual or legal entity that owns the activity or establishment. | If an individual, you must legibly write the full name and surnames of the owner of the company or business requesting the SOP; if a legal entity, indicate the full name and surnames of the legal representative. | | Box No. 15: Identity card or legal entity ID: | Write down the identity card number, as applicable. If the space is not sufficient, you may do so on additional sheets, including above all information regarding processes or procedures. | | Boxes No. 37 Generates hazardous waste | Indicate if hazardous waste is generated as a product of the operation of the company or activity. In this case, it must have a Waste Management Plan in accordance with the provisions of this Regulation. | | Box No. 38 Name of the technical director (in the case of health or related services) | It will only be filled out when the establishment has a director different from the owner. The person whose name is in this box must provide a photocopy of their professional title and certification from the respective professional association. This is the person authorized to exercise, by reason of their profession, the technical or scientific direction of the establishment. | | Box no. 39 Identity card number of the technical director | Write down the identity card number of the technical director.
