Each of the points included in that document is explained below to facilitate its completion. It is recommended to consult the full text of the Regulation to understand the context of application of said Reports.
Any additional questions that may arise when preparing the Operational Report can be clarified at the Unidad de Normalización of the Dirección de Regulación de la Salud or at the Health Governing Areas (Áreas Rectoras de Salud) or Regional Offices (Sedes Regionales) of the Ministry.
The information that the generating entities provide in the reports, as well as the records that support it, may be verified at any time by officials of the Ministry of Health.
1. GENERAL INFORMATION:
Generating entity (Ente generador): natural or legal person, public or private, responsible for the emission of atmospheric pollutants from indirect-type boilers and furnaces.
CIIU Code (Código CIIU): enter the number of the International Standard Industrial Classification (Código Internacional Industrial Unificado) currently used in the country for the activity served by the reported indirect-type boiler or furnace.
Sanitary Operating Permit Number (Número de Permiso Sanitario de Funcionamiento) and validity date: enter the certificate number issued by the Ministry of Health authorizing the operation of the establishment served by the indirect-type boiler or furnace and the date on which the Permit expires.
Exact Address: enter the exact address (province, canton, district, and street directions) of the generating entity.
Name and signature of the Technical Professional Responsible for the report (Nombre y firma del Responsable Técnico del reporte): Indicate the full name and signature of the professional to whom the generating entity has delegated the obligation to prepare the operational report.
Sampling Date (Fecha del Muestreo): enter the date the laboratory carried out the sampling.
Operational Report Date (Fecha del Reporte Operacional): enter the date of submission to the Ministry of Health.
Date and Number of the Laboratory Report (Fecha y número del Reporte de Laboratorio): enter the date the Laboratory contracted by the industry or company prepared the Laboratory Report, which must not have been issued more than three (3) months prior (article 20 of the Regulation). Also enter the number the laboratory assigned to the report.
Reported Period (Periodo reportado): Operational reports must be submitted with the following frequency:
Indirect-type boilers and furnaces A and B: two reports per year, one distributed during the first six-month period (semestre) and the next during the second six-month period. For the first six-month period, enter "From January 1 to June 31." For the second six-month period, enter "From July 1 to December 31." Indirect-type boilers and furnaces C and D: one annual report during the year between January and November.
For those generating entities that operate the regulated equipment for periods equal to or less than six months per year, they must submit a report within a period not exceeding thirty calendar days from the start of their annual operation.
2. TECHNICAL DATA FOR EACH INDIRECT-TYPE BOILER AND FURNACE:
. Category of indirect-type boiler and furnace (Categoría de caldera y horno de tipo indirecto): for boilers, in accordance with the provisions of Decreto Ejecutivo No. 26789-MTSS, Reglamento de Calderas, La Gaceta Nº 65 of April 2, 1998. Indicate whether it is category A, B, C, or D. For indirect-type furnaces, enter the category according to what is indicated in article 1 of this Regulation.
. Type of fuel (Tipo de combustible): specify whether it is búnker, diesel, mineral coal, LPG gas (GLP), IFOs, gasóleo, biomass fuel (bagasse, coffee husks, etc.) or others.
. Fuel consumption (Consumo de combustible): enter the daily consumption in cubic meters for each boiler. For solid fuels, enter the value in kilograms.
. Operating hours (Horario de operación): enter the daily start and stop times for each indirect-type boiler and furnace.
. Stack diameter (Diámetro de chimenea): enter the diameter in meters at the sampling point of the gases to the atmosphere if it is circular. For square or rectangular ducts, enter the width and length in meters.
. Total stack height (Altura total de chimenea): enter the height in meters from ground level to the point of gas discharge to the atmosphere.
3. LABORATORY ANALYSIS RESULTS PER BOILER:
. Note: The results of the sampled and analyzed emissions must be reported on a dry basis and referenced to five percent (5%) oxygen for boilers, and 7% for furnaces, using the following formulas:
Formula to transform emissions from wet basis (base húmeda) to dry basis (base seca):

EBH = Emission on a wet basis (Emisión en base húmeda).
% H2O = Moisture content of the gases.
Formula to correct dry basis gas emissions to the reference oxygen percentage:

Er = Emission calculated at the reference value.
Em = Emission measured on a dry basis.
Or = Reference level for O2 (5%, 7% or 10% as applicable).
Om = Measured value for O2.
. m3 (NTP) (normal conditions of pressure and temperature): refers to the requirement that volume calculations must be corrected to normal conditions, i.e., 101.3 kPa (760 mm Hg or one atmosphere) and 273.15 K (0°C).
. Measurement period (Periodo de medición): enter the total duration in hours of the sampling of the physical and chemical parameters, indicating the start time and end time.
. Distance: A = enter the length in meters to the sampling point from the stack discharge. B = enter the length in meters to the sampling point from the last flow restriction (elbows, valves, pumps, emission control equipment, etc.).
4. ANALYTICAL RESULTS FOR SULFUR AND NITROGEN (RESULTADOS ANALÍTICOS DE AZUFRE Y NITRÓGENO): Enter the concentrations of sulfur and nitrogen (percentage by mass) from samples of the fuel used during the sampling carried out by the contracted laboratory. This information does not apply to biomass fuels.
5. RECORD OF ACCIDENTS AND ANOMALOUS SITUATIONS (REGISTRO DE ACCIDENTES Y SITUACIONES ANÓMALAS).
All significant anomalous situations that have affected the operation of the indirect-type boiler(s) and furnace(s) or the control systems mentioned in article 10 of the Regulation will be explained in this section, e.g., preheating equipment for liquid fossil fuels, devices for analyzing gases resulting from the combustion process, emission control equipment, etc.
6. EVALUATION OF THE CURRENT STATE OF THE SYSTEM (BOILER(S), FURNACE(S), AND EMISSION CONTROL EQUIPMENT) (EVALUACIÓN DEL ESTADO ACTUAL DEL SISTEMA (CALDERA(S) HORNO(S) Y EQUIPO(S) DE CONTROL DE EMISIONES)).
A comment will be written in this section regarding the state of the boiler(s), furnace(s), and the control system(s) mentioned in point 5 above at the end of the reported period, based on the information from point 3 of this Guide: Laboratory analysis results per boiler and furnace, and on the inspections carried out by the professional(s) responsible for their operation and maintenance.
7. CORRECTIVE ACTION PLAN (PLAN DE ACCIONES CORRECTIVAS). If necessary, the Corrective Action Plan will be proposed in this section with the purpose of ensuring that emissions from the boiler(s) and furnace(s) comply with the emission values established in article 8 of the Regulation.
This Plan must include at least the following information:
. Activities to be performed.
. Start date and end date of each activity.
. Name of the person responsible for the activity on behalf of the generating entity.
. Observations.
8. ANALYSIS METHODS, SAMPLING METHODS, AND REFERENCE STANDARDS FOR EACH PARAMETER ANALYZED BY THE LABORATORY (MÉTODOS DE ANÁLISIS, MÉTODOS DE MUESTREO Y NORMAS DE REFERENCIA PARA CADA PARÁMETRO ANALIZADO POR EL LABORATORIO). In this section, the analysis methods, sampling methods, and reference standards used by the Laboratory contracted by the generating entity for each analyzed parameter will be noted. It is recommended to consult article 11 of the Regulation.
