INSTRUCTIONS FOR THE USE OF THE FORM ON THE NOTIFICATION Who must provide the information?
The exporter/generator must fill out boxes 1 to 9 and 13 to 22.
The carriers of the wastes must fill out boxes 10 to 12 (the first carrier must fill out box 10, the second box 11, and the last box 12; for further details see the instructions below).
The importer (if not done by the disposer) must fill out box 3. The disposer must fill out boxes 24 and 25. Boxes 26 to 28 are for use by customs offices. Circulation of the movement document.
Each consignment must be accompanied by a movement document. If it involves a general notification, a different movement document will be necessary for each consignment. It is recommended to attach a copy of the duly authorized notification to the movement document.
At the time of shipment, the carrier will fill out and sign box 10. The exporter/generator will file a copy.
Each successive carrier will do the same as the first carrier and fill out box 11 or 12 at the time of transshipment. The carrier delivering the consignment will keep a copy of the signed document for its files. If more than three carriers are involved in a given movement of wastes, an appendix containing the corresponding information will be attached to the document.
When the disposer has received the wastes, an authorized representative of the disposer will fill out box 24 and give a copy to the last carrier. The disposer will send a signed copy of the document to the exporter and to the competent authority of the State of export and, although the Basel Convention does not require it specifically, it is preferable to also send copies to the other competent authorities concerned.
Upon completion of the disposal of the wastes, the disposer will fill out box 25 of the document and send signed copies to the exporter and to the competent authority of the State of it is preferable to also send copies to the other competent authorities concerned. The disposer will file the original of the movement document.
B. Specific instructions for filling out the Movement Form (Anexo 2). The Basel Convention does not explicitly require that the shaded boxes be filled out. This information is required in accordance with the OECD System or the EC System or may have value for other reasons for the competent authorities.
Box 1: Provide the same information as in boxes 1 and 10 of the notification.
Box 2: Provide the same information as in box 2 of the notification.
Box 3: Enter the notification number to which the consignment refers. This must be copied from the upper right-hand corner of the notification. Indicate whether the movement is subject to a single notification or a general notification for multiple movements.
Box 4: For multiple movements, enter the serial number of the consignment in relation to the total estimated number of consignments indicated in box 4 of the notification; for example, write 4 of 11 for the fourth consignment out of the 11 foreseen in a general notification.
Boxes 5 to 7: Indicate the full name, address, and telephone and telex or fax numbers of each carrier, and the name, address, and telephone and telefax numbers of the contact person in case of emergency. Provide the information on the first carrier in box 5, on the second in 6, and on the last in 7. When more than three carriers are involved, the appropriate information on each must be attached to the form.
Boxes 8 and 9: Provide the same information given in boxes 8 and 9 of the notification.
Boxes 10 to 12: Indicate the mode of transport used (see codes in Anexo 3) and, where applicable, the registration or license number of the means of transport. Each successive carrier, or its representative, will sign the document upon receiving the consignment. The first carrier will fill out box 10, the second 11, and the last 12. When more than three carriers are involved, the appropriate information on each must be attached to the form.
The exporter will keep a copy of the signed form. At each transshipment of the consignment to a different carrier, the carrier delivering the consignment will keep a copy of the signed form. The first transshipment date must coincide with the date on which the transboundary movement actually began.
Boxes 13 to 16: Provide the same information as in the corresponding boxes of the notification.
Box 17: Enter the actual weight (in kg) or the actual volume (in liters) of the consignment transported and, where possible, attach copies of the weighbridge tickets. Some countries may require that the weight always be indicated.
Box 18: Indicate the type of packaging (see codes in Anexo 3) and the number of packages that make up the consignment.
Box 19: Provide the same information as in boxes 17, 18, and 19 of the notification.
Box 20: Indicate all special precautions that must be taken with respect to the consignment; for example, handling instructions from producers for their employees, health and safety information, including, among other things, how to deal with possible spills and transport emergency cards.
Use annexes if necessary.
Box 21: Indicate the date on which the consignment actually begins. This date should coincide with the first transshipment date indicated in box 10.
Box 22: At the time of shipment, the authorized representative of the exporter/generator must sign and date the movement document. The name of the authorized representative of the exporter/generator must also appear, in capital letters, next to the signature.
It should be noted that, by signing the declaration, the exporter and/or the generator certify that the information is complete and correct, as well as that a contract and the necessary financial guarantees and insurance exist. They also certify that all necessary authorizations from the competent authorities concerned have been received. Some countries may require that a copy or an original of the authorization from the competent authorities be attached to the movement document.
Box 23: Must be filled out by the importer if not done by the disposer. Include the quantity of wastes received, in kilograms and/or in liters, the date of receipt, the name of the importer, and the signature of its authorized representative. The name of the authorized representative of the importer must also appear, in capital letters, next to the signature. Also indicate whether the wastes have been accepted or rejected by checking the corresponding box. If for any reason the consignment is rejected, the importer must immediately contact its competent authority or authorities.
Upon receiving the wastes, the importer must give a signed copy of the movement document to the carrier. The importer will also send signed copies to the exporter and to the competent authority of the State of export. Some countries require that this certification be issued within three business days and that it be given also to other competent authorities concerned. The original of the movement document must accompany the wastes and, once transport from the importer to the disposer has been carried out, it must be signed by the carrier or carriers.
Box 24: Must be filled out by the authorized representative of the disposer upon receiving the transboundary consignment of wastes. Indicate the quantity of wastes received, in kilograms and/or in liters, the date of receipt, the name of the disposer, and the signature of its authorized representative. The name of the authorized representative of the disposer must also appear, in capital letters, next to the signature. Also indicate whether the wastes have been accepted or rejected by checking the corresponding box. If for any reason the consignment is rejected, the disposer must immediately contact its competent authority or authorities. Also indicate the date on which the disposal of the wastes will have been completed and the method used for it.
Upon receiving the wastes, the disposer must give a signed copy of the movement document to the carrier. The disposer will also send signed copies to the exporter and to the competent authority of the State of export. Some countries require that this certification be issued within three business days and that it be given also to other competent authorities concerned. Normally, the disposer will keep the original of the movement document.
Box 25: Must be filled out by the disposer to certify the completion of the disposal of the wastes. Indicate the date of disposal, the name of the disposer, and the signature of the authorized representative of the disposer. The name of the authorized representative of the disposer must also appear, in capital letters, next to the signature.
Signed copies of the form with box 25 filled out must be sent to the competent authority of the State of export.
Some countries require that this certification be issued within 180 days following the receipt of the wastes and that it also be sent to the other competent authorities concerned. Normally, the disposer will keep the original of the movement document.
Boxes 26 to 28: The Basel Convention does not require these to be filled out. They serve for control purposes of the customs offices at the borders of the countries of export, transit, and import. Some countries may require that the customs office at the border through which the wastes leave the territory send a copy of the movement document to the competent authority or authorities that authorized the transboundary movement of the wastes.
