- a)The identity of a person applying for an identity card for the first time must be corroborated by their parents, grandparents, siblings, or by two witnesses, identified with their identity card, who must be warned that, in case of false identification, they will incur the crime of falsification of a public document, defined in article 357 of the Penal Code.
If the person requesting an identity card is not registered on the electoral lists, the Registry shall include their name ex officio at the domicile indicated in the application.
- b)The identity card must be requested in person, on the forms or through the means provided by the Supreme Electoral Tribunal. The application shall be signed by the interested party who, in addition, must imprint at least one fingerprint (huella dactilar) of a finger from either hand, except in cases of absolute physical impossibility, which shall be recorded.
The signature shall not require authentication if the applicant identifies themselves with their previous identity card, even if it is verify the identity with Civil Registry data or information. Otherwise, the signature must be authenticated with the signature and seal of a lawyer or a public official authorized by the Tribunal.
The applicant shall be responsible for the truthfulness of the data provided in the application. The total or partial inaccuracy of essential elements for identification, besides causing the absolute nullity of the identity card issued based on that information, will cause the applicant to incur the crime of ideological falsehood, provided for and sanctioned by article 358 of the Penal Code.
- c)On a special form or through available means, a registered elector who changes domicile must apply before the Civil Registry for the transfer of their electoral registration to the new domicile; to do so, they must indicate their name and identity card number, the electoral district of their registration, and the district to which they wish to be transferred. In that application must also appear the fingerprint and the signature of the interested party, duly authenticated by a lawyer or by an authorized public official.
Regional offices and mobile identity card issuers (ceduladores ambulantes) shall be obligated to receive all applications for identity cards or elector transfers and send them immediately to the Civil Registry.
They must issue a receipt stating the time and date of receipt, on the forms that the Tribunal shall provide them.

(Sinalevi Note: By resolution of the Supreme Electoral Tribunal N° 2337 of September 6, 2004, this numeral was interpreted to the effect that: ".the requirement to appear accompanied by a relative or two witnesses when obtaining an identity card for the first time, is considered fulfilled for persons who have previously obtained the Minor's Identity Card (Tarjeta de Identidad de Menores, TIM) and carry it in good condition. For such purposes, as a security measure to be followed, the identity of the person may be proven by prior comparison of their fingerprint, without prejudice to any other security element or procedure determined by the Civil Registry, in case of doubt. Notify this resolution to the General Directorate of the Civil Registry and publish it in the Official Gazette La Gaceta".)
(By resolution N° 4404-E8-2016 of June 29, 2016, the Supreme Electoral Tribunal interpreted this article to the effect "that the requirement to appear accompanied by two witnesses when applying for an identity card for the first time is considered fulfilled for Costa Ricans by naturalization, since—in the procedure to obtain nationality—they proved their identity. The General Directorate of the Civil Registry shall carry out the necessary proceedings so that the officials handling application intake have the inputs to verify the identity of the naturalized citizen who presents themselves to apply for an identity card for the first time; the interested party must also present—at the time of the procedure—a personal document (for example, residence card, DIMEX, or other similar) or the respective Naturalization Certificate. The foregoing, without prejudice to any other security element or procedure determined by the Civil Registry, in case of doubt.")
