to the public system.
For the purposes of processing an operating permit with the Ministry of Health, AyA will issue a certification of the existence of the connection to the sewer system. For such purposes, interested parties must appear at the AyA Service Platforms and fulfill the following requirements:
- a)Application form, provided by AyA, fully completed and signed by the registered owner or their legal representative.
- b)A certified copy, original size and legible, of the cadastral map. If no cadastral map exists, the requirement must be substituted with a survey map that complies with the provisions of article 2, subsection q) of the Regulation to the National Cadastre Law.
AyA shall have a period of 20 business days to resolve the request.
INSTRUCTIONS FOR COMPLETING THE FORM
Boxes No. 1 through No. 5: Complete the information for the individual submitting the request for a Discharge Permit (owner or legal representative).
Box No. 6: Indicate the authority held by the person identified in Box No. 1 within the association, company, or foundation.
Boxes No. 7 through No. 13: Indicate the name, corporate name, legal identification number, email address, telephone number, fax number, and Service Identification Number (NIS) of the Generating Entity that is responsible for the discharge(s).
Box No. 14 through No. 19: Refers to the geographic location of the facilities where the wastewater originates; the address must include an easily identifiable reference point, such as a school, church, health center, police station, municipal buildings, and others, in addition to noting the respective number of streets and avenues.
Box No. 20: Briefly indicate the main activity of the Generating Entity.
Box No. 21: Refers to the International Standard Industrial Classification (Revision three), in which a code is assigned allowing the classification of the different economic activities.
Box No. 22: Indicate the daily, weekly, and monthly production schedule of the Generating Entity.
Box No. 23: Refers to the total number of employees present at the facilities, breaking them down into those working in production, administration, or other areas.
Box No. 24: Indicate the source of the water supply sources, both for human consumption and for processes, including the water flow rate required in each case and the total average flow rate for the last six months expressed in cubic meters per month (m3/ month). If the Generating Entity holds a water use concession, it must be clarified that the flow rate to be indicated does not correspond to the concessioned flow rate but rather to the actual flow rate used by the Generating Entity. In the event that the supply source corresponds to an aqueduct, the reported flow rate will be estimated based on the billed volume.
Box No. 25: Indicate the destination of ordinary wastewater, that is, the water generated by the activities of the employees (sanitary services, dining hall, washrooms, etc.).
Section III. GENERAL DATA OF THE PRODUCTION PROCESS OR COMMERCIAL ACTIVITY (in the case of generating entities not yet installed or established, the information must be entered based on projected or estimated data) Box No. 26: Indicate the stages and main raw materials used in the production process, as well as the name of the finished product(s).
Box No. 27: Indicate the quantity, units, and name of the finished product; this may be expressed in terms of the processed raw material.
Box No. 28: Indicate the months in which production is maintained at the processing plant or commercial activity.
Box No. 29: Indicate the discharge point of the special wastewater generated in the production processes.
Box No. 30: Refers to the average flow rate discharged during the last six months, expressed in cubic meters per month (m3/month).
Box No. 31: Indicate the average discharge time per day expressed in hours.
Box No. 32: Indicate the average number of days per month on which discharge occurs.
Box No. 33: Indicate the months of the year in which discharge occurs.
Box No. 34: Brief description of the wastewater pretreatment system.
Box No. 35: Note the actual or estimated values of the wastewater produced for each of the wastewater quality parameters requested by Decree No. 33601-MINAE-S.
Box No. 36: A flow chart must be attached indicating the main ordinary unit operations and those of the production process in which water is used. It may be done freehand or by computer; in both cases, it must be sufficiently clear, logical, and legible so as to allow for easy comprehension.
Note: If there is more than one discharge, the above information must be submitted for each one. In the case of Generating Entities that are in operation and do not have an Operating Permit, they must provide a copy of the analysis of the discharged wastewater quality, issued by an accredited laboratory (the analysis cannot be more than three months old).
36. Attach a flow chart in which the main ordinary unit operations and those of the production process in which water is used are indicated.
The applicant declares that the data provided are faithful and true and has given them under oath, likewise, commits to comply with all provisions of Decree 33601-MINAE-S (Regulation of Discharge and Reuse), and therefore, FORMALLY REQUESTS the Wastewater Discharge Permit to the Sanitary Sewer System administered by AyA.
