For the purposes of processing an operating permit with the Ministry of Health, AyA will issue a certification of the existence of the connection to the sewer system. For such purposes, interested parties must appear at the AyA Service Platforms and fulfill the following requirements:

• a)Application form, provided by AyA, fully completed and signed by the registered owner or their legal representative.
• b)A certified copy, original size and legible, of the cadastral map. If no cadastral map exists, the requirement must be substituted with a survey map that complies with the provisions of article 2, subsection q) of the Regulation to the National Cadastre Law.

AyA shall have a period of 20 business days to resolve the request.

INSTRUCTIONS FOR COMPLETING THE FORM

Boxes No. 1 through No. 5: Complete the information for the individual submitting the request for a Discharge Permit (owner or legal representative).

Box No. 6: Indicate the authority held by the person identified in Box No. 1 within the association, company, or foundation.

Boxes No. 7 through No. 13: Indicate the name, corporate name, legal identification number, email address, telephone number, fax number, and Service Identification Number (NIS) of the Generating Entity that is responsible for the discharge(s).

Box No. 14 through No. 19: Refers to the geographic location of the facilities where the wastewater originates; the address must include an easily identifiable reference point, such as a school, church, health center, police station, municipal buildings, and others, in addition to noting the respective number of streets and avenues.

Box No. 20: Briefly indicate the main activity of the Generating Entity.

Box No. 21: Refers to the International Standard Industrial Classification (Revision three), in which a code is assigned allowing the classification of the different economic activities.

Box No. 22: Indicate the daily, weekly, and monthly production schedule of the Generating Entity.

Box No. 23: Refers to the total number of employees present at the facilities, breaking them down into those working in production, administration, or other areas.

Box No. 24: Indicate the source of the water supply sources, both for human consumption and for processes, including the water flow rate required in each case and the total average flow rate for the last six months expressed in cubic meters per month (m3/ month). If the Generating Entity holds a water use concession, it must be clarified that the flow rate to be indicated does not correspond to the concessioned flow rate but rather to the actual flow rate used by the Generating Entity. In the event that the supply source corresponds to an aqueduct, the reported flow rate will be estimated based on the billed volume.

Box No. 25: Indicate the destination of ordinary wastewater, that is, the water generated by the activities of the employees (sanitary services, dining hall, washrooms, etc.).

Section III. GENERAL DATA OF THE PRODUCTION PROCESS OR COMMERCIAL ACTIVITY (in the case of generating entities not yet installed or established, the information must be entered based on projected or estimated data) Box No. 26: Indicate the stages and main raw materials used in the production process, as well as the name of the finished product(s).

Box No. 27: Indicate the quantity, units, and name of the finished product; this may be expressed in terms of the processed raw material.

Box No. 28: Indicate the months in which production is maintained at the processing plant or commercial activity.

Box No. 29: Indicate the discharge point of the special wastewater generated in the production processes.

Box No. 30: Refers to the average flow rate discharged during the last six months, expressed in cubic meters per month (m3/month).

Box No. 31: Indicate the average discharge time per day expressed in hours.

Box No. 32: Indicate the average number of days per month on which discharge occurs.

Box No. 33: Indicate the months of the year in which discharge occurs.

Box No. 34: Brief description of the wastewater pretreatment system.

Box No. 35: Note the actual or estimated values of the wastewater produced for each of the wastewater quality parameters requested by Decree No. 33601-MINAE-S.

Box No. 36: A flow chart must be attached indicating the main ordinary unit operations and those of the production process in which water is used. It may be done freehand or by computer; in both cases, it must be sufficiently clear, logical, and legible so as to allow for easy comprehension.

Note: If there is more than one discharge, the above information must be submitted for each one. In the case of Generating Entities that are in operation and do not have an Operating Permit, they must provide a copy of the analysis of the discharged wastewater quality, issued by an accredited laboratory (the analysis cannot be more than three months old).

36. Attach a flow chart in which the main ordinary unit operations and those of the production process in which water is used are indicated.

The applicant declares that the data provided are faithful and true and has given them under oath, likewise, commits to comply with all provisions of Decree 33601-MINAE-S (Regulation of Discharge and Reuse), and therefore, FORMALLY REQUESTS the Wastewater Discharge Permit to the Sanitary Sewer System administered by AyA.

Para los efectos de tramitar un permiso de funcionamiento ante el Ministerio de Salud, el AyA otorgará una constancia de la existencia de la conexión al sistema de alcantarillado. Para tales efectos, los interesados deberán presentarse a las Plataformas de Servicio de AyA y cumplir con los siguientes requisitos:

• a)Formulario de solicitud, proporcionado por AyA, completo y firmado por el propietario registral o su representante legal.
• b)Una copia certificada del tamaño original y legible del plano catastrado. En caso de que no exista plano catastrado, el requisito debe ser sustituido por un plano de agrimensura, que cumpla con lo estipulado en el artículo 2 inc. q) del Reglamento a la Ley de Catastro Nacional.

El AyA contará con un plazo de 20 días hábiles para resolver la solicitud.

INSTRUCCIONES PARA COMPLETAR EL FORMULARIO Apartado I. DATOS DEL SOLICITANTE (PROPIETARIO O REPRESENTANTE LEGAL) Casillas Nº 1 a la Nº 5: Completar la información de la persona física que realiza la solicitud de Permiso de Descarga (propietario o representante legal).

Casilla Nº 6: Indicar la potestad que tiene la persona indicada en la casilla Nº1 dentro de la asociación, sociedad o fundación.

Apartado II. DATOS GENERALES DE LA ACTIVIDAD Casillas Nº 7 a la Nº 13: Indicar el nombre, razón social, cédula jurídica, correo electrónico, teléfono, fax y Número de Identificación de Servicio (NIS) del Ente Generador que se responsabiliza del (los) vertido(s).

Casilla Nº 14 a la Nº 19: Se refiere a la ubicación geográfica de las instalaciones en donde se originan las aguas residuales, la dirección deberá contar con un punto de referencia fácilmente identificable, como por ejemplo una escuela, iglesia, centro de salud, estación de policía, edificios municipales y otros, además de anotar el número de calles y avenidas respectivas.

Casilla Nº 20: Indicar brevemente la actividad principal del Ente Generador.

Casilla Nº 21: Se refiere a la Clasificación Industrial Internacional Uniforme (revisión tres), en la cual se asigna un código que permite la clasificación de las diferentes actividades económicas.

Casilla Nº 22: Indicar la jornada de producción diaria, semanal y mensual del Ente Generador.

Casilla Nº 23: Se refiere al número total de empleados que permanecen en las instalaciones, desglosando los que trabajan en producción, administración u otros.

Casilla Nº 24: Indicar la procedencia de las fuentes de abastecimiento de agua, tanto para consumo humano como para procesos, incluyendo el caudal de agua requerida en cada caso y el caudal total promedio del último semestre expresado en metros cúbicos mensuales (m3/ mes). En caso de que el Ente Generador posea concesión de aprovechamiento de aguas, se debe aclarar que el caudal que se debe indicar no corresponde al caudal concesionado sino al caudal real utilizado por el Ente Generador. En caso de que la fuente de abastecimiento corresponda a un acueducto, el caudal reportado se estimará a partir de volumen facturado.

Casilla Nº 25: Indicar el destino de las aguas residuales ordinarias, es decir, de las aguas generadas por las actividades de los empleados (servicios sanitarios, comedor, lavatorios, etc.).

Apartado III. DATOS GENERALES DEL PROCESO PRODUCTIVO O ACTIVIDAD COMERCIAL (en caso de entes generadores aún no instalados o establecidos, se deberá ingresar la información a partir de datos proyectados o estimados) Casilla Nº 26: Indicar las etapas y principales materias primas utilizadas en el proceso productivo, así como el nombre del o los productos terminados.

Casilla Nº 27: Indicar la cantidad, las unidades y el nombre del producto terminado, se puede expresar en términos de la materia prima procesada.

Casilla Nº 28: Indicar los meses en los cuales se mantiene la producción en la planta de proceso o actividad comercial.

Casilla Nº 29: Indicar el punto de descarga de las aguas residuales especiales generadas en los procesos de producción.

Casilla Nº 30: Se refiere al caudal promedio vertido durante el último semestre, expresado en metros cúbicos por mes (m3/mes).

Casilla Nº 31: Indicar el tiempo promedio de vertido al día expresado en horas.

Casilla Nº 32: Indicar el promedio de días al mes en que hay vertido.

Casilla Nº 33: Indicar los meses del año en los cuales existe vertido.

Casilla Nº 34: Breve descripción del sistema de pretratamiento de aguas residuales.

Casilla Nº 35: Anotar los valores reales o estimados de las aguas residuales producidas para cada uno de los parámetros de calidad del agua residual solicitados por el Decreto Nº 33601-MINAE-S.

Casilla N° 36: Se debe adjuntar un diagrama de flujo en el que se indiquen las principales operaciones unitarias ordinarias y de proceso productivo en las que se utiliza el agua. Se podrá realizar a mano alzada o en computadora, en ambos casos deberá ser lo suficientemente claro, lógico y legible de tal manera que permita su fácil comprensión.

Nota: en caso de existir más de una descarga, se deberá presentar la información anterior para cada una de las mismas. En caso de Entes Generadores en operación y que no cuenten con Permiso de Funcionamiento, deberá aportar copia del análisis de la calidad del agua residual vertida, emitido por un laboratorio habilitado (el análisis no puede tener más de tres meses de realizado).

36. Adjuntar un diagrama de flujo en el que se indiquen las principales operaciones unitarias ordinarias y de proceso productivo en las que se utiliza el agua.

El solicitante manifiesta que los datos consignados son fieles y verdaderos y los ha otorgado bajo fe de juramento, asimismo, se compromete a cumplir con todo lo dispuesto en el Decreto 33601-MINAE-S (Reglamento de Vertido y reúso), por lo tanto, SOLICITA formalmente el Permiso de Descarga de Aguas Residuales al Sistema de Alcantarillado Sanitario administrado por el AyA.
