throughout the entire text - Full Text of Regulation 331 Application forms for authorization to purchase fuel at a competitive price, signature registration card for natural persons and legal entities, and form for submission of applications to INCOPESCA for processing in group mode Full Text of record: 11D75C COSTA RICAN INSTITUTE OF FISHERIES AND AQUACULTURE AJD1P/331-2017.-Puntarenas, on the eleventh day of August, two thousand seventeen.
I.-That by Board of Directors agreement AJDIP-330-2016, this collegiate body approved the Regulation for the Regulation and Control of the Efficient Use of Fuel at a Competitive Price, intended for the national fishing fleet.
II.-That with the publication of the cited Regulation, it is necessary to readjust the forms for: Application for Authorization to Purchase Fuel at a Competitive Price, Signature Registration Card for natural persons, Signature Registration Card for legal entities, and Form for Submission of Applications to INCOPESCA for Processing in Group Mode.
III.-That Mr. Julio Dijeres Bonilla, head of the fuel section, presents to the Board of Directors the proposed forms for the purpose of being approved; for which reason, the Board of Directors, Therefore, AGREES:
1º-To approve the forms for Application for Authorization to Purchase Fuel at a Competitive Price (Annex 1), Signature Registration Card for natural persons (Annex 2), Signature Registration Card for legal entities (Annex 3), and Form for Submission of Applications to INCOPESCA for Processing in Group Mode (Annex 4).

I.-Que mediante el acuerdo de Junta Directiva AJDIP-330-2016 se aprobó por este órgano colegiado el Reglamento para la Regulación y Control del uso eficiente del combustible a precio competitivo, destinado a la flota pesquera nacional.
II.-Que con la publicación del Reglamento citado es necesario readecuar los formularios de: Solicitud de Autorización de Compra de Combustible a Precio Competitivo, Tarjeta de registro de firmas para personas físicas, Tarjeta de registro de firmas para personas jurídicas y Formulario de entrega de solicitudes a INCOPESCA para su tramitación en modo grupal.
III.-Que el señor Julio Dijeres Bonilla, jefe de la sección de combustible, presenta ante la Junta Directiva la propuesta de formularios con la finalidad de ser aprobados; razón por la cual, la Junta Directiva, Por tanto, ACUERDA:
1º-Aprobar los formularios de Solicitud de Autorización de Compra de Combustible a Precio Competitivo (Anexo 1), Tarjeta de registro de firmas para personas físicas (Anexo 2), Tarjeta de registro de firmas para personas jurídicas (Anexo 3) Formulario de entrega de solicitudes a INCOPESCA para su tramitación en modo grupal (Anexo 4).
2º-Los formularios: Factura de venta del producto desembarcado, Declaración Jurada días de Pesca, Constancia de traslado de información cuando la embarcación descargue en un lugar diferente a su base de operación, Formulario de Informe de Viaje para la Flota Pesquera Turística, Convenio para trámite y Obtención de Combustible a precio Competitivo para la Flota Pesquen no Deportiva para personas físicas, Convenio para trámite y Obtención de Combustible a precio Competitivo para la Flota Pesquera no Deportiva para personas físicas y la Boleta de control de gasto de combustible a precio competitivo, no se incluyen en este acuerdo por cuanto no sufren modificaciones.
