Amendment to ESPH Water and Sanitation Services RegulationReforma al Reglamento de Servicios de Agua y Alcantarillado de la ESPH

• 1)A literal and updated certification issued by the Property Registry (Registro de la Propiedad) or by a Notary Public.
• 2)If it is a new construction, the cadastral map of the property and an official copy, or a copy authenticated by a notary public, of the construction permit issued by the competent authority.
• 3)The interested party must complete and sign the service request form and, if it is approved, must pay the value of the connection, the fees, and the charges stipulated therein.
• 4)Present their identity card (cédula de identidad) and submit the respective photocopy of said document, on both sides. When the submission of the request is made by a third party, it will be sufficient for the signature of the interested party to be duly authenticated.

• a)Complete the corresponding disconnection form.
• b)Prove that they are current in the payment of services by presenting the respective receipt(s).
• c)Pay in advance the invoice corresponding to the current period.
• d)The form must be signed by the person whose name appears on the receipt, or failing that, by the current owner of the property, in which case they must provide a literal certification of ownership issued by the Registry (Registro) or by a Notary Public.
• e)The applicant must present their identity card (cédula de identidad) and provide a photocopy of it on both sides.

The Company (Empresa) will not process disconnection requests for any service for houses or premises inhabited by tenants, without their express consent being recorded, in accordance with the provisions of Article 9 of the Ley General de Agua Potable.

• a)Complete the transfer request form, indicating a telephone number, fax, or address for notifications.
• b)Present their identity card (cédula de identidad) and submit the respective photocopy, on both sides.
• c)A literal and updated certification issued by the Property Registry (Registro de la Propiedad) or by a Notary Public, for the property in question.
• d)Presentation of the payment receipt(s) for the aqueduct or sewerage service(s) for the months prior to the request date, as applicable, duly paid. In the event of not having the receipts, they must attach an explanatory note, to the satisfaction of the Company (Empresa), giving an account of the reason for said omission.

Said request shall indicate: a telephone number, fax, or address for notifications as applicable, and the interested party must present their identity card (cédula de identidad) and provide a photocopy of the document on both sides. When the submission of the request is made by a third party, it will be sufficient for the signature of the interested party to be duly authenticated.

In order to request a temporary service, the interested party must:

• a)Submit the corresponding written request, indicating a telephone number, fax, or address for notifications.
• b)Present their identity card (cédula de identidad) and submit the respective photocopy, on both sides.
• c)A literal and updated certification issued by the Property Registry (Registro de la Propiedad) or by a Notary Public, for the property in question.

The corresponding request must be submitted to the New Services Section, also expressing: the type of service requested; the place where the service is to be used, the duration of the activity, and the estimated number of users who will use the service.

When dealing with services for civic festivals, public spectacles, and other activities of this nature, the authorizations and permits required from the competent authorities must be attached.

When the request comes from one of the current clients of ESPH, they must prove they are current with the payment of services and other obligations they have with the Company (Empresa) and authorize the cost of consumption exceeding the deposit given as a guarantee to be included in their billing.

2.-It becomes effective upon its publication in the Official Gazette La Gaceta.

• 1)Certificación literal y actualizada expedida por el Registro de la Propiedad o por Notario Público.
• 2)Si es construcción nueva, el plano catastrado del inmueble y copia oficial o autenticada por notario público del permiso de construcción expedido por autoridad competente.
• 3)El interesado deberá llenar y firmar la fórmula de solicitud de servicio y si la misma fuera aprobada deberá cancelar el valor de la conexión, los derechos y las tasas ahí estipuladas.
• 4)Exhibir su cédula de identidad y presentar la fotocopia respectiva de dicho documento, por ambos lados. Cuando la presentación de la solicitud la hiciere un tercero, bastará con que la firma del interesado se encuentre debidamente autenticada.

• a)Llenar la fórmula de desconexión correspondiente.
• b)Comprobar que se encuentra al día en el pago de los servicios con la presentación del recibo o recibos respectivos.
• c)Cancelar por adelantado la factura correspondiente al período en curso.
• d)El formulario deberá ser firmado por la persona cuyo nombre aparece en el recibo o en su defecto por el dueño actual del inmueble, en cuyo caso deberá aportar certificación literal de propiedad expedida por el Registro o por Notario Público.
• e)El gestionante deberá exhibir su cédula de identidad y aportar su fotocopia por ambos lados.

La Empresa no tramitará solicitudes de desconexión de ningún servicio de casas o locales habitados por inquilinos, sin que conste su expreso consentimiento, de conformidad con lo dispuesto en el artículo 9º de la Ley General de Agua Potable.

• a)Llenar la fórmula de solicitud de traspaso, indicando número telefónico, fax o dirección para notificaciones.
• b)Exhibir la cédula de identidad y presentar la fotocopia respectiva, por ambos lados.
• c)Certificación literal y actualizada expedida por el Registro de la Propiedad o por Notario Público, del inmueble de que se trata.
• d)Presentación del (los) comprobante (s) de pago de servicio o servicios de acueducto o alcantarillado de los meses anteriores a la fecha de la solicitud, en su caso, debidamente cancelados. En caso de no contar con los comprobantes deberá acompañar nota explicatoria a satisfacción de la Empresa dando cuenta del motivo de dicha omisión.

Además deberá indicar el lugar en el cual está ubicada la propiedad a cuyo servicio se refiere la certificación y sus datos de identificación.

Dicha solicitud indicará: Número telefónico, fax o dirección para notificaciones en su caso, debiendo el interesado exhibir su cédula de identidad y aportar fotocopia del documento por ambos lados. Cuando la presentación de la solicitud la hiciere un tercero, bastará con que la firma del interesado se encuentre debidamente autenticada.

A fin de solicitar un servicio temporal, el interesado deberá:

• a)Presentar la solicitud escrita correspondiente, indicando número telefónico, fax o dirección para notificaciones.
• b)Exhibir la cédula de identidad y presentar la fotocopia respectiva, por ambos lados.
• c)Certificación literal y actualizada expedida por el Registro de la Propiedad o por Notario Público del inmueble de que se trata.

La solicitud correspondiente deberá ser presentada a la Sección de Servicios Nuevos, expresando además: Tipo de servicio solicitado; lugar donde se desea utilizar el servicio, tiempo que durará la actividad y cantidad estimada de usuarios que usarán el servicio.

Cuando se trate de servicios para fiestas cívicas, espectáculos públicos y demás actividades de esta naturaleza, deberán acompañarse las autorizaciones y permisos requeridos de las autoridades competentes.

Cuando la solicitud proviniere de alguno de los clientes actuales de la ESPH, éste deberá comprobar encontrarse al día en cuanto al pago de los servicios y demás obligaciones a su cargo con la Empresa y autorizar se incluya en su facturación el costo del consumo que excediere al depósito dado en garantía.
